In today’s healthcare environment, hospital stays and post-surgical recovery are more condensed than they once were. Carlinville Area Hospital’s Transitional Care Unit helps to fill the gap between hospitalization and home.
TCU is a Medicare covered stay for those who qualify for a skilled service which includes therapy services (PT, OT, ST), IV antibiotics or skilled wound care services. Patients who come to TCU for rehab or IVs enjoy spacious private rooms, accessible bathrooms and showers, a friendly, welcoming staff and a plan of care focused on helping patients achieve their goal of returning to home. Interdepartmental rounds and care overseen by their primary care physician or hospitalists includes individualized care and therapy services are 6 days per week.
Our TCU continues to provide comprehensive short term rehabilitation for our community members who are recovering from illness or surgery and require inpatient therapy services to restore their independence and safety before returning home. Patients benefit from recovering in their own community where visits from family and friends are encouraged and Activities staff help to provide purposeful activities throughout their stay in TCU.
Comprehensive discharge planning begins at admission to TCU and may include coordination of necessary post-hospital resources, equipment, or in- home safety evaluations by OT to ensure patients have what they need to safely return home. If needed, patients will also be referred for outpatient or Home Health services for optimal continuity of care.
