Case Manager
Position Summary:
The Case Manager facilitates the coordination of services and care designed to transition patients through the continuum of care. This is accomplished through effective intervention and communication with the patient, family, caregivers, payers and other members of the healthcare team.
Department: Care Management
Status: Full-Time; Non-Exempt; Days; 40 hours per week
Pay Range: $32.00 - $48.00 / Hour
Position Requirements:
- Graduate of an accredited school of nursing or Licensed Social Workers. Bachelor's degree preferred.
- Current IL RN License in good standing or current IL registered Social Worker.
- At least one year of Case Management experience required
- Current BLS (or willingness to obtain)
