WHAT IS PATIENT PORTAL?

The Patient Portal is a secure web-based system that will allow you to access portions of your medical record. Once you have registered for the Patient Portal, this service is available online anytime you want to log in.

 

HOW DO I GET STARTED?

· The next time you are at CAH for a test or procedure, show the Admitting staff your photo ID, give them your personal email address, and review and sign the Consent Form. You will receive an email invitation with a link to our Patient Portal and Registration process.

· Click the link within the email, it will take you to the website where you are able to register for a Patient Portal account. You will have the opportunity to create your own secure User ID, Password, and Security Questions.

· Now you can begin accessing your medical records online. For security reasons, remember to keep your Password and Security Questions and Answers private.

· For future visits to the CAH Patient Portal, go to our website at www.cahcare.com and click on the Patient Portal link on our Home page.

 

WHAT IS NEEDED TO ACCESS THE PATIENT PORTAL ?

All you need is a computer, laptop, tablet, or smartphone with internet access and an email address.

 

CAN I REGISTER FOR THE PORTAL OVER THE PHONE?

No. For security reasons, no personal information will be accepted over the phone.

 

WHY CAN’T I ACCESS MY CHILD’S ACCOUNT THROUGH THE PATIENT PORTAL?

For children under 18, limited Patient Portal accounts will be available. Privacy Laws prevent parental access to certain portions of a child’s records. For copies of your child’s Medical Record, contact our Health Information Management (HIM) Dept. @ 217-854-3141 x291, during Regular Business Hours, Monday – Friday, 8:00 am – 4:30 pm.

 

I AM HAVING TROUBLE LOGGING IN AND ACCESSING THE PATIENT PORTAL. WHAT DO I DO?

Please contact our Information Technology (IT) Dept. during Regular Business Hours, Monday – Friday, 8:00 am – 4:30 pm., @ 217-854-3141 x350. They can assist you in accessing your Patient Portal Account. If your call is received after Regular Business Hours, leave a message and contact phone number. Your call will be returned on the next Regular Business Day.

 

WHAT IF I FORGET MY PASSWORD?

Log on to the Patient Portal and Click on the FORGOT PASSWORD Link. Follow the Directions.

 

SOME OF MY MEDICAL INFORMATION IS WRONG. HOW DO I CORRECT THIS?

Regarding your Medical Information, our goal is to have an interactive partnership with our patients. We believe that the Patient Portal will help us keep your information correct. If you believe that there is an error within your Personal Medical Information, please contact us at 217-854-3141 x291, during Regular Business Hours.

 

WHY CAN’T I SEE ALL OF THE INFORMATION IN MY MEDICAL RECORD?

At this time, only certain Medical Information is available on the Patient Portal. For additional copies of your Medical Information, please contact our Health Information Management (HIM) Dept. @ 217-854-3141 x291, during Regular Business Hours. For all questions regarding tests and results, please contact your Primary Care Physician.
